    It’s so hard to get a real idea of what happened, when we read these very basic articles.

    A caregiver at an Urbandale nursing home has been charged with wanton neglect after being accused of physically abusing an elderly resident.

    Chad Lombard, 24 , is accused of using force to restrain a 94-year-old woman in her bed at USA Healthcare, 4614 N.W. 84th St . The Nov. 17 incident allegedly resulted in bruises and abrasions to the woman.

    Lombard and the administrator of USA Healthcare could not be reached for comment Wednesday.

    Court records indicate Lombard has been ordered to have no contact with the woman. He has been a certified nursing assistant for at least three years. A conviction on the misdemeanor charge of wanton neglect would make him ineligible to continue working in an Iowa care facility.

    The Iowa Department of Inspections and Appeals, which regulates nursing homes, is investigating the matter, said department spokesman David Werning.

    In 2003, the Urbandale home was fined $67,795 for taking two residents off oxygen ventilators without a physician’s order. One of the residents died. Last year, the home was fined $350 for alleged neglect that resulted in a resident’s thumb being amputated. This year, federal officials asked state inspectors to increase their oversight of the home.
